Policies
Your understanding of the counseling process is important to us. Our understanding of the purpose of your visit is equally important.
To help with this, we ask you to fill out two forms before your first visit to your counselor. You may do this by:
1.) opening the following forms and printing them out to filled out and brought with you to your first visit, or
2.) coming about 15 minutes early and filling out the forms in our waiting area.
Your personal information is protected by state law. On your first visit your counslor will review these two forms with you to be sure that you understand your rights and responsibilities as a client of CrossRoads as well as your counselor's responsibilities to you. The biographical form will help your counselor become aware of the personal goals and issues that have led you to seek counseling.
