The door you should enter for your counseling appointment is on the north side of the building. Please make yourself comfortable in chairs there and our staff will come out to meet you at your appointment time.

Paperwork
Your understanding of the counseling process is important to us. Our understanding of the purpose of your visit is equally important. All of the paperwork you will need is available below. You have the option of downloading the paperwork and filling it out before you come or we will have the forms sitting out for you in the waiting area. The forms take about 15 minutes to fill out, so please arrive early if you plan to fill them out in the office.
Your personal information is protected by state and federal laws. On your first visit your counselor will review the forms with you to be sure that you understand your rights and responsibilities as a client of CrossRoads as well as your counselor's responsibilities to you. The biographical form will help your counselor become aware of the personal goals and issues that have led you to seek counseling.
